Psychosocial Recovery Coach

Centre of Cares Psychosocial Recovery Coach is an Advanced practice nurse in Mental Health with (Master of Nursing). Advanced Specialised Knowledge in Mental Health with Completed Advanced studies in Cognitive Behavioural Therapy and Advanced Therapeutic Interactions. 

A psychosocial recovery coach is a specialist mental health support person that some NDIS participants with mental health conditions may have access to under their plans. 

The role of a recovery coach is to assist people with disabilities arising from mental health conditions to live a full and contributing life.

They will bring specialist skills and knowledge to help people with psychosocial disabilities take control of their lives and navigate both the NDIS and the mental health system.



Support Co Ordination

Centre of Care’s Support Co Ordinator will work with you closely to identify your needs, personalise supports and make sure you are connected with the right services. We regularly check to make sure you are continually getting the best support and assessing if

any changes or adjustments are required to achieve your goals.

DURING SUPPORT CO ORDINATION MEETINGS, WE WILL:

  • Implement and monitor your plan
  • Identify services that meet your individual needs
  • Co-ordinate service providers and support.
  • Organise manner of timing of supports, including service agreements and obtaining quotes.
  • Connect you into work or other community activities.
Social Support & Community Participation

We provide assistance to support you participate in the community. For most people staying connected with family and the community is important, having a disability doesn’t mean you have to miss out.

So whether it is:

  • Taking an art class
  • Volunteering
  • Going to a social event
  • Shopping or just doing something fun

 Centre of Care can support you in living a busy, full and exciting life.

Accommodation & Tenancy

Centre of care aims to ensure you obtain or retain appropriate accommodation this may include:

 

  • Assistance to apply for a rental tenancy

 

  • Undertake tenancy obligations.
Domestic Assistance & Household Tasks

Centre of Care staff are happy to help you do things around the house just the way you like. 

Some of our many domestic assistance services range from:

  • Changing bed linen
  • Laundry
  • Vacuuming
  • Cleaning the fridge
  • Gardening
  • Meal preparation and delivery. 

Your Centre of Care Support Person can assist you with your shopping and put it away for you.

Community Nursing Service & Personal Care

Centre of Care offers a community nursing service, it is imperative we deliver quality nursing care and management on an ongoing basis. Our community nursing service will assist with:

  • Assessing your clinical and personal care needs
  • Medication management
  • Personal care such as hygiene
  • Grooming
  • Mealtime support needs.

Personal care is an important element regarding your ability to live independently at home. 

Complex care needs will be referred to the appropriate health service provider in order to be delivered if not already in place. Our clients are guaranteed, caring, respectful attention which is centred around wellbeing, dignity, and privacy. Whatever your medical need, it is a great peace of mind having a professional visit your home.

Transport Arrangements

There is always things that need doing. For example:

  • Transport to get to an appointment
  • Going shopping
  • Visit a friend
  • Go to the movies
  • Go to the bank
  • Medical appointments
  • Picking up your new hearing aids or glasses.

Centre of Care will support you with these tasks and in order to maintain your independence.

All our vehicles have comprehensive insurance and drivers hold a current driver’s license.

Independent Living 

Centre of Care provides assistance in daily living and life skills either in independent or shared living environments. Skill-building helps manage things such as:

  • Literacy
  • Budgeting
  • Household duties training
  • Food preparation and nutrition.
Mental Health Support & Counselling

Our Centre of Care team has registered Mental Health Nurses that are experienced with an extensive range of knowledge in mental health conditions with different levels of complexities.

Our Nurses can visit you in your own home, offering mental health assessments, support, and individual counselling. We can work in collaboration with your GP or Psychiatrist contributing to care planning.

So, whether it is in your own home, via telephone consult or external appointment with an allied mental health professional, Centre of Care can arrange for you to have your therapeutic support where you are most comfortable. Support and mental health education is also offered to carers and families. Wherever you are in your journey we will support you in reaching your goals.
